This can cause gaps in frontline clinical time for consultations but also in a reduction in leadership capacity within practices. • National policy demands the provision of Primary Care 8am-8pm 365 days a year. • There is an increasing number of elderly and more complex patients needing care in the community. • There are differences in the quality of services between different GP practices in Wandsworth. • There are significant health inequalities within the borough. • The existing infrastructure (IT & estates) are not always fit for purpose to deliver high quality care.
